Fatal Ontario Industrial Accident on Cocamonga Avenue
ONTARIO, Calif. (August 5, 2021) — Alfredo Munoz, 54, of Los Angeles, died Tuesday morning in an industrial accident in the 1600 block of South Cucamonga Avenue in Ontario.
Also, the incident happened about 9:12 a.m. on August 3rd, 2021, according to the San Bernardino County Sheriff Coroner.
Additionally, emergency crews with the Ontario Fire and Police Departments responded to the 1600 block of S. Cucamonga Avenue. Upon arriving at the scene, they located a construction site with a crane that dug a large hole. The report does not mention exactly what happened to Alfredo Munoz, but it appears he was working here.

Alfredo Munoz suffered major injuries in the incident. He was pronounced dead at the scene at 9:18 a.m., the report said.
Meanwhile, investigators with the Ontario Police Department are looking into the circumstances. Anyone with information that may help authorities determine the cause of his death may contact Ontario Police.
