Fatal Madera Car Accident on Avenue 15 near Road 28 1/2

MADERA, Calif., (October 5, 2026) — A car accident involving multiple vehicles led to a fatality and a DUI arrest Sunday night on Avenue 15 near Road 28 1/2 in Madera, the California Highway Patrol reported.

Police arrested a 32-year-old man following the two-vehicle crash that left another man dead at the scene.

According to the incident report, the crash occurred on Oct. 4th at approximately 6:03 p.m. and involved two white vehicles—a Nissan Altima and a Mercedes van. The vehicles reportedly collided with a brick wall, resulting in one vehicle overturning. The scene was described as chaotic, with debris scattered and all lanes blocked. Emergency services, including CAL FIRE and EMS, were promptly dispatched to the scene.

Jamie Hernandez, a passenger in the victim’s vehicle, was taken to a children’s hospital, where a CT scan was pending to assess the extent of injuries.

Law enforcement officers from the Madera County Sheriff’s Office are actively investigating the circumstances of the crash. They are urging anyone who may have witnessed the incident to come forward and provide any information that could aid in the investigation.
